To enable us to provide our services to individual clients it is often necessary for us to obtain information about them that may be of a personal, sensitive or confidential nature. This usually includes personal contact details, and may include information relating to the individual’s health and daily living activities and habits.

In providing our services to you, it may be necessary for us to provide personal information obtained from you to other organisations with whom we conduct business.

Under the Act you have the right to access personal information we hold about you. An application may be made in writing for access to such information only by the individual about whom the information is kept. If the information is incorrect, you have the right to require us to amend the information.
